Evaluation
Database (EXCEL file) -- "The Evaluation Database
is a reporting tool into which data from evaluation forms
can be entered. It's purpose is to track indicators over time
and measure mentor programs' success on an outcome."
Mentor
Evaluation (PDF file) -- "This evaluation form is completed by
the mentor at six months into the relationship and at its
conclusion. It is used to report changes that a mentor has
observed in the young person. The results from this form are
entered into the Logic Model and used for reporting purposes." (click

Younger

younger children in a mentor relationship to complete. The

- Needs Assessment Form - The
Needs Assessment Form is used to determine what services
mentor programs need most, to make referrals, and to see how VMP
can best support the mentor programs of Vermont. If you run a

- E-Mentoring, also known as online mentoring, describes
a mentoring relationship conducted via the Internet. It's goals
are similar to face-to-face mentoring: establishing a trusting,
positive relationship between a caring adult mentor and a young
person. E-Mentoring also helps young people deepen their understanding
of the positive potential of online culture and high-tech communications.
Most online mentoring programs use e-mail while some use Web-based
programs or special communications software available only to
those involved in the program. While most E-Mentoring involves
a one-to-one relationship similar to traditional mentoring relationships,
some programs use a group-to-group mentoring format, such as group
of professional engineers advising a classroom of students. For

- Elements of Effective Practice
- These Elements of Effective Practice
present program policies and practices that have proven effective
in a wide range of existing mentoring settings. The brochure is
available to any individual or organization that wants to develop,
operate, volunteer in or fund a mentoring program. Designed and
endorsed by the National Mentoring Partnership, The Vermont Mentoring
Partnership has adopted these elements as their Quality Assurance
Standards.
